Commonwealth Corporation encourages all approved* grantees and vendors to use Electronic Funds Transfer (EFT) payments to reduce the instance of check fraud. Grantees/vendors can fill out the fillable form below to authorize Commonwealth Corporation to deposit funds into a bank account.

This form will allow you to setup a new account, update an existing account, or cancel an old one.

*In order to receive reimbursement of training costs, you must have an agreement on file. All applications for funding must be submitted at least 21 days before the start date of training and must be approved.
