This form should be completed by a representative authorized to sign on behalf of the company (e.g., Human Resources representative, the CFO or COO, etc.). This person will be asked to verify general company information, such as FEIN and total number of W-2 employees, and to upload supporting documentation related to the grant itself, such as a payroll summary report verifying that all employees taking training are being paid as W-2 employees.
