What will I be asked for on the Express monitoring form?

The Express monitoring form has 5 parts: 


  1. Page 1 asks respondents to verify the name of an authorized representative of the company who can complete and sign the form on behalf of the company. Typically, managers or senior-level company executives (CEO/Partner, Chief of Human Resources, CFO, COO, etc.) are authorized signatories. 
  2. Page 2 asks respondents to verify the business information and number of MA W-2 employees at the time of application. The page also provides an opportunity to update the business address on file. 
  3. Page 3 asks respondents to: 
    • Confirm the courses, training providers, and award amounts of their grants. 
    • Provide a short response addressing the impact of the training on the skills and productivity of the employees that have been, are being, or will be trained with Express-funded training. 
    • Upload a list of the first and last names of all trainees that have taken, are taking, or will take training and upload a payroll summary report verifying that these trainees are paid as Massachusetts-based W-2 employees. 
    • Certify that the indicated employees are, have been, or will be paid their regular wages when Express-funded training is being delivered. 
  4. Page 4 asks the respondent to confirm that no self-dealing has occurred through this training grant. It also provides the opportunity for respondents to request support before submitting the form if there are questions.  
  5. At the end of the form, respondents will confirm their responses and sign to authorize the form.
